  You're liking running NLB and having the perfect scenario for them to be hitting one node half the time. Need to isolate the node first then start fishing the IIS logs. Likely have an app in iis failing or a corrupt directory. Worst case, rebuild a CAS frontend, join it to the farm, and swap the IPs out.
