Custom emojis in message body are not screenreader friendly

Custom emojis in the message body are invisible to screenreaders, meaning they get skipped over. There should be alt text included with the emoji. Ideally there would be an additional field in emoji settings where guild admins can submit descriptive text for the emojis, but defaulting to the emoji name would go a long way as well.
