Add maximum volume option

An option to set a per-client maximum volume option would be divine.

A close friend of mine has a hearing disability that makes him extremely sensitive to loud noises - it causes physical pain for him, so much so that a small server we have has a rule specifically saying to not be loud when he's in voice chat. However, when we were in a larger, public server, someone decided it'd be funny to enter voice chat and scream. My friend was hospitalized after this, for nearly a full week, right before Christmas.

Additionally, as the owner of a reasonably large server, this feature would be useful in moderating public voice channels for when people get a bit loud.
