Mod form

There should be a built in mod-form for an administrator to add moderators easily. Like have something under community settings called "Mod Form" where the admins and/or owner can make a mod form and go in and automatically add the moderator role if the user sounds like a good fit.

So instead of having to make a google form, I can just have a built in one and easily add moderators.
