Add Membership Screening results to Audit Log

Would like to have Membership Screening results where they have read and agreed to the rules included in the audit log with their user ID and date at a minimum.

Having these records are important for enforcing a click wrap agreement should the terms come up in court. With Discord being used more for commercial purposes click wrap records are very important.
