Discord support ticket form not showing most of the boxes to tick

my account recently got disabled for being underaged even tho I’m not, so I sent an email and a ticket to appeal and get my account back. but after watching a few youtube videos on how to do it properly I see that they have a bunch of boxes to ticks, stuff among the lines of :”are you in the right place?”, “have you read ToS and community guideline before submitting the ticket”, … while I only have a phone number box, subject and description to fill. the first 2 images are people I see’s and the other 2 down below are mine
