Ghost notifications

I have EVERY server im in muted, I have ALL notifications turned off. yet I get on discord, and i get what i've begun calling ghost notifications. I will be playing a game and all the sudden I start hearing the notification noise over and over and over again, but when i go look there is nothing new in any server and no private messages (which are also muted). The ONLY thing i don't have muted are phone call notifications but hilariously I don't hear when someone is calling me. why is this happening?
