Thread Titles
I feel like it's really weird to have to require a title for every thread. It should be optional. It should just be an extension of the comment which spawned the thread which might cover multiple topics or be vague or just not need a title. I don't know how else to say it; it's just weird and annoying having to add a title to all of my conversations. The initial comment should speak for itself to describe the ensuing thread.
I guess it's probably to have something to show in the left sidebar? Frankly, I think they just shouldn't be in the left sidebar. It's cluttered, and they're not channels. Especially once 20 or so threads have started, it's just obnoxious having them there. Having them accessible via the thread button in the top bar (in which you can display the initial message like a pinned message without a title) or via clicking on the the initial message is perfect. That said, if they absolutely must be in the left sidebar for some reason, then you could just display the first few words with an ellipsis in slightly smaller-than-normal and slightly-greyed-out text if/when they're started without a title.

The only reason I can come up with is that threads are shown in the sidebar, so there's going to be limited space to refer to the thread which a potentially multi-paragraph message is unsuitable for. I agree with the sentiment, however, I don't think they're necessary. They should be optional, or at least just default to a truncated version of the root message.
I think the choice to display threads in the sidebar is questionable in the first place.
