Discord Forms - New Feature Suggestion

Hello Discord Team,

id like to suggest a new feature which would be usefull for many developers & discord communitys.

This could be used for
Ticket Bots & Customer Support
Communitys & Surveys
Bann & Mute Appeals
Securely send Information without using public/private channel

All Possible Fields which could be added:

Field Types:
Checkbox
Small Textbox
Large Textbox
Numbers Only

Features:
(If set) Text,Number Field have to match a specific Regex.
Placeholder for specific Fields.
Default Value for specific Fields.
Make Checkbox, Number/Textfield to be Requiered or not.
Multiple-Choice-Questions
Upload Files
Choose Date/Time in which time this "Discord Form" can be filled
Add Webhook URL where to send these Info.
