Do Discord hides fields that we need to check?!

Everyone that is creating a ticket seem to have a bunch of “CONFIRM” in their “Ticket description” that never show up when creating the ticket. But certain Discord employees close the ticket within minutes of them being created and mark these “CONFIRM” fields as “no”.
Why is Discord hiding these? Is it to be able to meet a quota to close tickets? Are these fields only required on certain tickets? Without feedback from Discord it is hard to know. I managed to make them visible again. I will create a new ticket with these checked and see if the system accept that I checked these fields.
Maybe then I can get proper support with my disabled account.
One of these hidden fields are “CONFIRM: My account was NOT disabled for being too young to use Discord.” which seems like something I WANT to check when doing an appeal.
If someone is more knowledgable about this system they are free to chime in if I am totally off on this.

Here is the form with what I feel are the most relevant of these CONFIRM options checked and enabled. There were APP ID and SERVER ID available as well, but those feel more related to API development and server management.

I haven't submitted a new ticket yet. Still need to add a picture with me holding my ID card. My discord username, and I will include my email as well, there was no field to enter this in this field.
The original form looks like this:

Which is very different from what seem to be the correct form?
Why would for example the two top confirm option be hidden if they seem to be required?

It accepted the input so I could confirm these various things. But nelly didn't care, Nelly closed the ticket within minutes anyway.
