Show the user an optional form to fill when leaving a server regarding their reason of departure

When managing a server that promotes chatting and making new friends, there will always be people exhibiting bad or toxic behavior that could ruin the community or make members not feel welcomed and this may lead to the member leaving abruptly the server

Since not everyone knows the tools a server can have for reporting problems (pinging the staff, opening a ticket with a Ticket bot, contacting the administrator, ...) as well as some people simply don't care about reporting problems, the staff or the server administrator will not be made aware of the issue that lead to that departure.

Therefore I think it should be important for discord members to fill an optional form when leaving a server, either customizable by the server administrator like the popup when joigning a community server, or the same for everyone : simply a text box and a title saying "If you so desire, please tell us the reason of your departure" or something like that.
This would make discord server/communities much more healthy in my opinion
